Root causes: (1) an effect ordering bug in `usePdfMeetingGeneration` was clearing form update refs on every streaming chunk instead of only on generation start, (2) the final form update used stale React state instead of the freshly parsed data, (3) auto-create could fire before the form was populated. Fixed by splitting the lifecycle effect, passing final parsed data directly, and gating auto-create on sections being present.
  • **Document editor title syncs to document name** — Editing the inline title heading in the document editor now also updates the document name when it still matches the default "Namnlöst dokument" or the previous title, so the breadcrumb and drive table reflect the new name immediately
